Only 28.9% of healthcare workers exposed to bullying file a formal report (Averbuch et al., systematic review of 68 studies, n=82,349). The primary deterrent: fear of career implications (41.1%). 30.7% of victims report information-withholding affecting performance as a bullying behaviour: voice suppression as active hierarchical control, not passive fear [Averbuch]. The gradient is steepest where authority is most concentrated. Among targets, 54.6% of women report bullying versus 34.2% of men. 43.6% of women report missed career opportunities versus 18.8% of men [Averbuch]. Among Italian nurses exposed to horizontal violence, 87.4% show physical and psychological symptoms; ~10% meet PTSD criteria [Bambi]. Experimental work confirms the mechanism: brief exposure to rudeness reduces cognitive task performance in direct targets and in witnesses [Porath & Erez]. Observers of rude interactions are more likely to perpetrate rudeness themselves [Foulk et al.] the contagion that reproduces the behaviour across cohorts.
