1986年10月2日のチェルノブイリでのMi-8ヘリ墜落後、乗員4人全員（パイロット含む）が即死。原因はクレーンケーブルへの接触によるもの。機体は原子炉近くに墜落し、残骸は排除区域に埋められた。清掃作業は続き、11月に石棺が完成した。
