#תשובות_לשאלות והפעם... על תפקידו של KRAS בסרטן ריאות אז באו נצא לדרך! 🧵 חלבון K-Ras פועל כ״מתג מולקולרי״ בינארי במסלול RAS/MAPK: קושר GTP (פעיל) ↔ GDP (כבוי) תחת בקרה של GEFs (SOS1) ו-GAPs. מוטציות סומטיות בעיקר קודונים 12/13/61 משתקות את ה-GAP-binding, גורמות לאקטיבציה קבועה (constitutive signaling) של RAF→MEK→ERK ו-PI3K/AKT, ומקדמות פרוליפרציה, הישרדות, נדידה וגרורות. כשהמוטציות הנפוצות הן: G12C, G12D, G12V אשר משנות חומצת אמינו באתר הקטליטי, חוסמות סטרית את GAPs ומשאירות את KRAS נעול ב-GTP-bound state. כתוצאה מכך נוצר איתות בלתי פוסק אשר גורם עמידות לאפופטוזיס, שינויים בציטוסקלטון, הגברת invasion/metastasis. ב-NSCLC (אדנוקרצינומה) שכיחות KRAS ~15-30%, כאשר G12C (הנפוצה ביותר) מהווה 30-40% מהן (~13% מכלל NSCLC adenocarcinoma) וקשורה ישירות לעישון (signature G>T transversion). בהשוואה לגידולים אחרים: • PDAC: ~90% KRAS (G12D/V/R) • CRC: 30-45% (G12D/V, G13D) • NSCLC adenocarcinoma: 15-30% (G12C דומיננטית) G12C נדירה בחולים שלא עישנו השכיחות בישראל ומוערכת בכ-13% מחולי NSCLC קיימות מוטציה מסוג G12C ״החלבון הבלתי ניתן לטיפול״ Undruggable עד שנת 2019 מוטציה ב KRAS נחשבה בלתי ניתנת לטיפול בגלל: 1. זיקה פיקומולרית ל-GTP (ריכוז מילימולרי בציטופלזמה). 2. מבנה ״כדור גולף״ חלק ללא כיסי קישור עמוקים. ניסיונות בעבר עקיפים (FTI, MEK/ERK inhibitors, synthetic lethality) נכשלו ברובם עקב רעילות ו/או עמידות. פריצת הדרך הראשונה הגיעה עם מעכבי G12C בהם גילוי ציסטאין ייחודית ב-G12C המאפשרת עיכוב אלוסטרי קוולנטי בלתי הפיך במצב GDP-bound. אשר ״נועלים״ את KRAS במצב כבוי, מונעים שחלוף ל-GTP. שתי תרופות קבלו את אישור הFDA בשנים האחרונות: • Sotorasib (AMG510 / Lumakras) • Adagrasib (MRTX849 / Krazati) נתונים ממחקרים קליניים: CodeBreak 100 & KRYSTAL-1 Sotorasib: ORR 37.1%, DCR 80.6%, mPFS 6.8 , mOS 12.5 Adagrasib: ORR 42.9%, mPFS 6.5 , עם יתרון בטיפול בגרורות מוחיות BBB (iORR 33.3% ). בהמשך מחקר CodeBreak 200: PFS 5.6 vs 4.5 ח’ לעומת docetaxel (HR 0.66). לציין שמעכבי KRAS G12C שומרים על יעילות גם ב-STK11-mut וב-PD-L1 נמוך, אך פרוגנוזה שלילית ועמידות חלקית עם KEAP1 co-mut אך כאן אנו נתקלים באתגר העמידות נרכשת אשר מופיעה במהירות בערך כ mPFS ~6 לאחר התחלת הטיפול בעיקר במנגנונים הבאים: 1. On-target (secondary mutations ב-KRAS: Y96D, allele switching ל-G12D/V). 2. Off-target bypass (EGFR/MET amp, RAF/MEK/PI3K hyperactivation). 3. Histologic transformation (ADC → SCC/SCLC). 4. TME alterations.
