Yes I would disagree. The static/cidal enthusiasts have been making these claims for many years across many drugs. The first RCT that disproved this nonsense was the ampho/flucon RCT for candidemia (academic.oup.com/cid/article…). Until that point, it was verboten to give azole ampho.

So, why did the guidelines say to county Day 1 with neg. blood cultures? Because the RCT of ampho vs. flucon to treat diss. candid. was designed with a 14 day course of therapy, from first day of neg blood cultures. PMID 12746765, published in 2003, during the guideline process.

We follow the RCT. 1 dose 10 mg/kg AmBisome 5FC followed by flucon 5FC.

The flucon vs ampho RCT was 1994, and the IE guidelines published in 1995 without the first neg blood culture thing. Only after the candida guidelines were published in 2004 did IE guidelines switch.

Predni-pos kapky jsou steroidy. Steroidy jsou i v jiných očních kapkách ( Flarex, Dexamethasone, Efflumidex, Flucon..). Je to nepříjemnost, ale není pravda, že se nedají nahradit.

Fluconazole just has much less early fungicidal activity vs AmpB - not helped much with higher doses, seem 1200mg or if adding 5-FC does approach efficacy of AmpB and possible improved survival - helps prevents flucon-R emergence via heteroresistance (@DrNeilStone !)

No dec. in cases in Sub-Saharan Africa outside SA, despite ART; IDSA standard of 2w AmpB 5-FC based on studies from 1990s e.g. nejm.org/doi/full/10.1056/ne… but AmpB is hard to find in many countries, needs IV, hard to administer and monitor - Flucon monotherapy not as effective
