見えない貧困を映すもの　～2ドルの眼鏡が変えるインドの未来～｜瀧波 一誠 ☕️ World-Building Analyst （地理学・世界観分析家） @mokosamurai777 #地理がすき note.com/mokosamurai/n/nac0f… ◎26/6/6 「インド東部オディシャ州の都市、ブバネーシュワル。野菜売りのトファン・ジェナさんは、テントの下で人生初の視力検査を受け、新しい眼鏡を手にしました。その瞬間、これまでぼやけていた文字や遠くの景色が、彼の目に鮮明に飛び込んできました。彼の体験は、WHO（世界保健機関）が指摘する『予防可能、または未対応の視覚障害を抱える世界10億人以上』の問題を象徴しています。彼が手にした眼鏡は高度な医療技術の結晶ではなく、2ドル未満で提供されたとても安価なものです。では、この『比較的安価で効果が高い道具』が、なぜいまだに数億人規模の人々に届いていないのでしょうか。このニュースは、『見えることへの医療アクセス格差』として格差と貧困の現状を示すものの一つです。 インドで今、何が起きているのか？ ジェナさんが眼鏡を手にしたのは、ドイツ発のNGO『GoodVision』がインドの現地パートナーを通じて貧困地区に設営した、移動型の視力検査キャンプでのことでした。最新の推計には幅がありますが、GoodVisionの説明等ではインド国内で約5億5000万人が視力矯正を必要としており、約2億人が眼科ケアにアクセスできている一方、約3億5000万人は十分なアクセスを持たないとされています。他の報道などでは未アクセス人口を約2億5000万人とする推計もありますが、いずれにせよ数億人規模のアクセスが十分にできない人が存在することは確かです。GoodVisionのモデルは、材料費を約1ドルに抑え、地域の所得水準に合わせて低価格で提供する点に特徴があります。レンズは大量生産品を使用し、フレームはインド国内において金属ワイヤーを電力不要の簡易な曲げ機で加工して作られています。同団体はインドで60万件を超える視力検査と24万本超の眼鏡提供を行い、約150人の現地人材を雇用・育成してきました。さらに重要なのは、検査の中で白内障などの疾患が見つかった患者を提携病院へ紹介し、手術支援につなげている点です。移動キャンプが、高度医療への最初の窓口ともなっているのです。 なぜ、この『アクセス格差』が起きているのか？ ① 『技術』ではなく『距離と機会』の壁 眼鏡が届かない最大の理由は技術の不足ではなく、視力検査を受けられる場所までの移動距離です。世界最大の眼鏡レンズメーカーであるEssilor社の指摘によれば、理想的な光学店の比率は1万人に1店舗とされていますが、インドでは『約7万人に1店舗』しか存在しません。さらに言えば、これは全国に平均的に存在するわけではなく、大都市の中心部には高度な眼科医療が集中する一方で、都市周縁のスラムや農村部には初期診療のサービス網が極端に薄いのです。さらに、価格だけでなく『認識と時間』も大きな壁です。自分の視力低下が矯正可能だと知らない、検査を受けるために仕事を休めばその日の収入が減るなど、日銭労働の制約や情報不足も、ラストマイルを阻む要因となっています。 ② 『商材扱い』の壁と、オディシャ州の挑戦 インド政府も無策ではありません。『失明予防・視覚障害対策の国家プログラム』や一次眼科ケア指針のもと、学校での視力スクリーニングや地域保健人材（ASHA）を通じた支援の仕組みは存在します。しかし、14億人を超える人口規模に対して公的な制度だけでは不十分です。多くの中低所得国では、眼鏡は医療インフラではなく『小売業の商材』として扱われがちです。そのためサービスが利益の出る都市の中間層以上に偏り、低所得層が排除されやすい傾向があります。そんな中、ニュースで取り上げられているオディシャ州は、インドで初めて約60億ルピー（約100億円）規模の『包括的眼科プログラム（UEHP）』に先駆けて取り組んだ州でもあります。州政府の制度的な厚みとGoodVisionのようなNGOの機動力が連動することで、貧困層へのアクセスがようやく確保され始めているのが実態です」
