That entire screenshot *screams* mental illness. So fucking obvious. Any doctor that performs that sorta surgery without a full psychological workup (think Autism) from a proper panel needs their license revoked, and jail time. Tired of hearing/reading about negative results.

We welcome @CMSGov proposals which are spot on! Time to remove patient care barriers, increase access, streamline workup/treatment, get rid of redundant practices that lead to increased health care $. No reason to still practice same as 20 years ago. @SCAI

Just made another MGUS diagnosis today. About one every week now l and only in the multi- Covid jabbed. Appears as a mild anemia and an SPEP is part of my usual anemia workup besides the usual labs and checking for GI bleed causes.

What long covid highlighted for me is that I was left undiagnosed or under-treated for conditions that covid made much worse, like a long-term fibromyalgia diagnosis without ever having a hypermobility workup, like it turbocharged everything I was told to just live with.

What should a doctor do in such a case? Initial workup: • CBC with differential • Peripheral smear (look for blasts) • Reticulocyte count • LDH & uric acid (high cell turnover) • Coagulation profile (PT/INR, aPTT, fibrinogen) • Renal & liver function tests • Electrolytes • Chest X-ray if mediastinal involvement suspected Confirmatory tests: • Bone marrow aspiration/biopsy • Flow cytometry (immunophenotyping) • Cytogenetics & molecular studies Important: Do not label these patients as simple gingivitis or viral illness without checking CBC if red flags are present: • Pallor • Fever • Bruising/bleeding • Lymphadenopathy • Hepatosplenomegaly • Bone pain Early suspicion saves lives.

A new review in Current Opinion in Pulmonary Medicine highlights how lung cancer screening diagnostic efficiency can be improved across three key domains: 📌 Eligibility and uptake 📌 Adherence to annual screening 📌 Diagnostic evaluation of concerning findings Low-dose CT screening has reduced lung cancer mortality in major trials, but real-world implementation remains complex. The goal is not only to detect early-stage lung cancer. It is also to avoid diagnostic harm.
