While some may view ARISE-FLUIDS RCT (NEJM) as the final blow to the 30 mL/kg recommendation, a new cohort study just published in JAMA of 25,481 patients found that initial administration of ≥30 mL/kg was associated with lower 30-day mortality among patients with hypoperfusion or intermediate lactate elevation without severe cardiac or kidney comorbidities. Data is clear there is no advantage of IV therapy to oral for GNB bacteremia, as long as the pt can tolerate po and source is controlled.
