A large group from the @EosinophilSoc including @EosDoc @adklion @TPongdee and others published retrospective data on biologic use in #HES. Covered during the last @AIMedEdJC #AIMedEdJC Covered more here 👇🏾 x.com/KhouryMD/status/151506…
📈⬆️ interest in using biologics for patients with eosinophilic disorders One main issue: ‼️ Most not approved or have warnings for #eosinophil associated diseases like HES ✅ Clinical workgroup of the @EosinophilSoc set up multicenter databases to answer some key Qs A 🧵

Thanks @somewheresy for the great teaching point! Eosinophil counts quickly drop (often to zero) after oral/iv corticosteroids. Do NOT use a normal eosinophil count to rule out eosinophilic disease without verifying a person was not on corticosteroids. #AIMedEdJC #MedTwitter

The effects of corticosteroids can linger for a few weeks. If an eosinophilic disorder is suspected & it is safe to stop corticosteroids, an eosinophil count can be checked again after a few weeks to see if there is an elevated baseline level. #AIMedEd #AIMedEdJC #eosinophil I

Thanks for bringing this up, @karenrobbinsmd and thanks for the initial post, @winder_gill! Great article to take a look at👇 relevant to this week’s #AIMedEdJC topic
I think there’s other data out there to support this….am recalling a @ckeet @aolayink article where amongst publicly insured individuals, “White patients accounted for a disproportionate amount of biologic treatment” #AIMedEdJC pubmed.ncbi.nlm.nih.gov/3355…

Agree and I have had the opposite occur….re check and AeC is markedly elevated #AIMedEdJC
Re-check eos is a big one! I've gotten a few consults with only one screen > 1500 and on repeat it was normal. #AIMedEdJC

Great point @winder_gill! The differential for eosinophilia is huge. Allergist-immunologists are always happy to help but also appreciate if you are also considering non-allergic causes 😀 #AIMedEdJC

I think there is a larger 'takehome' message not just for us, but for #primarycare and those involved with our patients - We often see eosinophilia on labwork, and yes, it can be associated with atopic disorders, but please consider the differentials! #AIMedEdJc @AIMedEdJC

If you can make the eos go down some with skin care...then more likely AD associated eosinophilia. If no matter what you do to the skin eos remain elevated and refractory skin dz then probably other way around. No easy answer but clinical judgement #AIMedEdJC

Philosophical question: in pt a AD w elevated eos do they have AD w elevated AEC as part of the disease or do they have HES manifesting as AD? #AIMedEdJC
