ALT A PBUB is a rare but often fatal complication of endoscopic variceal band ligation. The relevant risk factors contributing to PBUB are unknown and consequently there are no effective preemptive strategies. This study highlights kidney disease as a predictor of PBUB together with the well-established higher risk for urgent band ligation.

ALT This study reviews existing research on the use of GLP-1 receptor agonists in managing gastrointestinal diseases and finds that early evidence suggests these agents hold promise across multiple conditions, supporting the case for gastroenterologists to serve as prescribers.

ALT In a US cohort of 21.3 million deliveries, women with coexisting IBD and CLD had significantly higher odds of preterm birth, hypertensive disorders of pregnancy, perinatal mood disorders and other complications compared with either condition alone, highlighting the need for risk-based CLD screening and multidisciplinary pregnancy care.

ALT This secondary analysis of the MAESTRO-NASH trial examined the effects of resmetirom in patients with weight loss and/or treated with therapies for T2D. The efficacy of resmetirom was not impacted by background T2DM therapies. Weight loss (≥ 5%) enhanced the effect of resmetirom on multiple MASH endpoints.

ALT irAEs, particularly dermatologic, endocrine and low-grade events, were associated with improved ICIs effectiveness in HCC. Early irAEs detection and corticosteroid management preserve survival benefits. However, more large-scale prospective studies are needed to verify our findings.

ALT The CROHN-PREP trial compared a simplified clear-liquid preparation to a standard PEG-based regimen in Crohn's disease patients before SBCE. Both ensured similar intestinal cleanliness and diagnostic yield, while patient acceptability was almost perfect and twice as high with the simplified preparation. ALT Poor psychological health persists for a substantial number of patients with IBD. Anxiety trajectories established at 3-month intervals over the course of a year are significantly associated with future symptoms of anxiety, and depression trajectories with future symptoms of depression during two years of follow-up.

ALT In a large-scale, multiple-site Danish faecal microbiota transplantation (FMT) system, the cure rates in patients with Clostridioides difficile infection treated with encapsulated FMT were improved following exclusion of three low-performing donors and implementation of a two-dose FMT regimen. Following multivariable mixed-effects analysis, the impact of two-dose FMT remained significant.

ALT In-depth multiomics analyses revealed that the gut microbiota in patients with CPH is dysbiotic. Altered gut microbiota plays an important role in the progression of CPH and affects the host's arginine metabolism to promote arginine biosynthesis. The pro-inflammatory effects of certain gut bacteria induce iNOS expression, which together act on the arginine–NO pathway to promote NO synthesis and exacerbate CPH.

ALT This study investigated multiple non-invasive tests (NIT) in a phase 2b study of patients with MASH and fibrosis treated with semaglutide. Findings revealed that semaglutide significantly improved NIT scores versus placebo from as early as 28 weeks. The results support the use of NITs to assess a treatment response to semaglutide.

ALT This systematic review shows that while endoscopy, computed tomography and magnetic resonance imaging generate substantial carbon emissions in inflammatory bowel disease (IBD) care, intestinal ultrasound (IUS) offers a clinically validated, low-carbon alternative. Adoption of IUS can align high-quality IBD management with planetary health goals by reducing the environmental footprint of diagnostic imaging.

ALT In a multi-centre study of obeticholic acid in primary biliary cholangitis, 45% discontinued the drug over 4 years, with 11% moving to combination therapy (fibrates). Of those continuing, response rates increased from 37% (1 year) to 55% (4 years). Non-response at 12 months and cirrhosis were independent predictors of liver-related events.

ALT This systematic review and meta-analysis of 34 studies including 1586 patients with severe alcohol-associated hepatitis found pooled mortality rates of 26.8% at 28 days, 35.1% at 60 days, and 43.7% at 90 days. Cumulative Bayesian analyses showed no credible improvement in short-term mortality across the past four decades.

ALT No significant differences were found in adherence or relapse of either a single 5-ASA tablet or three-tablet treatment. The simplified single-tablet lower dose treatment could be a feasible alternative to the conventional three-tablet regimen.

ALT This study evaluated the performance of the PAGE-B score in HCC risk among CHB patients with metabolic dysfunction or MASLD. The PAGE-B score effectively stratified HCC risk in both MASLD and non-MASLD patients, especially in those without cirrhosis, where low scores were linked to negligible 5-year HCC risk.
