🫁 Can early ctDNA help predict survival in NSCLC treated with radiotherapy? A new Clinical Cancer Research study suggests that it can. Hashmi and colleagues evaluated ctFE, circulating tumor fraction estimate a machine learning-derived metric of ctDNA burden in plasma. The study focused on patients with locally advanced and oligometastatic NSCLC treated with radiotherapy-based approaches. Key findings: 🧬 Baseline ctFE strongly stratified survival 📌 OS: HR 5.93; P = 0.005 📌 PFS: HR 11.08; P < 0.001 ⏱️ Mid-treatment ctFE, measured during days 10–14, remained highly prognostic 📌 OS: HR 7.08 📌 PFS: HR 12.06 📌 Both P < 0.001 ctFE also outperformed Max VAF and simple ctDNA detectability. Most strikingly, early ctFE dynamics defined 3 molecular response groups with very different median overall survival: ✅ Favorable response: 60.8 months ⚠️ Intermediate response: 13.0 months 🔴 Poor response: 2.9 months 📌 P < 0.001 The biomarker was also validated in two external cohorts: 🏥 Locally advanced NSCLC treated with chemoRT: n = 94 🏥 Oligometastatic NSCLC treated with RT: n = 309 The takeaway: Early ctDNA burden may provide a practical, noninvasive way to identify high-risk patients during treatment, before conventional imaging can fully define response. This could support more personalized treatment adaptation in locally advanced and oligometastatic NSCLC. ‼️ Landmark #lungcancer research published today in Cell Scientists at the Francis Crick Institute and UCL have discovered a 14-protein blood signature that predicts lung cancer more than 5 years before diagnosis, works in never-smokers, and could identify who benefits from prevention treatment. People with high levels of these proteins were the ones who actually benefited from a drug to damp that inflammation down, cutting their lung cancer risk nearly in half. This could finally give us something lung cancer has never had: a way to prevent it in the people most at risk. doi.org/10.1016/j.cell.2026.… #LCSM
