Massive goiter (left) versus a control thyroid (right)!! The goiter is due to inactivation of the thyroglobulin gene. It compensates for inefficient T4 production in dead thyrocytes, thereby keeping T4 levels normal. Kudos to the Arvan group!! pubmed.ncbi.nlm.nih.gov/4180…

We followed 105 patients before and after developing hypothyroidism (on LT4). Those patients with lower T3 were at risk for dyslipidemia (47.5% vs. 27.3%); even after adjusting for confounders (adjusted OR: 3.6). Monitoring T3 levels should be considered.

Over a 20-year follow-up, patients receiving combination therapy (LT4 T3 or DTE) had a 27% lower risk of dementia and a 31% lower risk of mortality compared to those receiving LT4 alone. Adjusted Cox proportional hazard models confirmed the results.

Last Thursday I had the pleasure of speaking at Kuma Hospital in Kobe, Japan. Profs. Ito and Akamizu explained that they keep thyroidectomized patients on LT4 and a subnormal TSH to normalize T3 levels and minimize residual symptoms, with very positive results overall.

Look at these new and exciting findings! Some areas of the brain (e.g., the cerebellum) in LT4-treated mice remain hypothyroid even after TSH levels have normalized. Could this contribute to the brain fog reported by many patients on LT4?
