If we don’t refer uncomplicated hypothyroidism to endocrinology, or refer mild hypertension to cardiology, why are we referring vasomotor symptoms out? Menopause care belongs in primary care.

Same hormones. Very different roles. Contraceptives suppress ovulation with high doses of "synthetic" hormones, but MHT aims to replace missing hormones at physiologic levels. It is the indication, history, dose, route, and life stage that determines risk, not just hormone type.

Dose ≠ effect! Early COCs used >50 mcg ethinyl estradiol, but modern COCs use 20–35 mcg and remain effective because, pharmacologically, EE is highly potent. MHT often uses higher milligram doses because the estrogens used are weaker. Clearly, potency and clinical intent matter.

Know your options. “Hormones” ≠ pills. Both contraception and MHT can be delivered orally, transdermally, vaginally, or via devices. Same hormones, different delivery, different clinical trade-offs. Route matters.
