Contraindications to ECMO include irreversible processes such as metastatic cancer, active bleeding, aortic insufficiency (for peripheral venoarterial only), and cases of contraindications to anticoagulation (aneurysms, bleeding disorders, and strokes). #emergencymedicine

Left-Sided Ductal Dependent CHD: Presents 1-2 weeks after ductus arteriosus closes w/ poor feeding, lethargy, irritability, and eventually shock. Hypoplastic left heart presents w/ cyanosis while critical aortic stenosis & coarctation of the aorta are acyanotic. #EMBoards

Kawasaki syndrome (cont'): Labs will show elevated LFTs, ESR/CRP, WBC, and Platelets. Manage w/ IVIG high dose aspirin (80-100 mg/kg/day). Most cases are self-limited within 4 weeks while 20% of cases have complications (coronary artery aneurysms) #emergencymedicine #EMBoards
