The good answer is A. Yes. The creation of the Lightning Flow token is a sub-event of the prevention, and happens before the original event occurs (6.4.1a) Because it exists before it begins to occur, its prevention effect can be applied to Tap Lessons Past's remaining damage.

The answer is B — Game Loss. Per PPG 3.6, passing a pack the wrong way is a Minor violation only if discovered before the next pick. Discovered after, it irreversibly compromises the draft and becomes a Major violation.
