Retrograde VA jump in second slide- maybe could provoke slow-fast AVNRT? At shorter coupling intervals, infrahisian conduction delay (longer VH) leading to gap phenomenon and shortening of HA interval.

Interesting ECG. My guess is orthodromic AVRT with alternating conduction over fast and slow pathway (QRS alternans). There is also a “pseudo” electrical alternans due to rapid heart rate.

Yup - they were on flecainide! Just a simple example of the use-dependence nature of Class I antiarrhythmics and specifically, the QRS prolongation seen with Class IC drugs.

Agree with all of you. Sinus brady and AIVR with AV dissociation (during sleep). There is evidence of fusion in 2nd, 3rd, and last beat of this telemetry strip. I've labeled the P-P intervals - sinus rate slowly increases and the last sinus beat is able to conduct with fusion.
