New Video on JACEP Open: A transit passenger with unilateral leg pain
A 57-year-old man with recent trauma and long flight presented with severe thigh pain and sepsis, initially concerning for DVT.
POCUS ruled out DVT and identified a fluid collection within the hamstring, consistent with pyomyositis. He improved after antibiotics and surgical drainage, highlighting the role of ultrasound in diagnosing musculoskeletal infections.

New in JACEP Open: Prevalence of Exposures and Moral Injury in First Responders
In a survey of 292 first responders, exposure to potentially morally injurious events (PMIEs) was common, with 18.4% of those exposed reporting clinically meaningful moral injury. Moral injury symptoms showed strong correlations with PTSD, anxiety, depression, and burnout, underscoring the mental health risks faced by first responders.
