💬 Editorial: In a randomized trial, aerobic exercise and intensive vascular risk reduction for older adults at risk of #dementia led to only modest cognitive improvement, with no significant group differences in global cognitive function. ja.ma/3Pvi2Nl

Retracted #RCTs linked to superretractors or top-cited authors were published earlier, retracted later, and accumulated more citations than other retracted trials. ja.ma/4fbogwc

💬 Viewpoint: Routine exclusion of pregnant individuals from RCTs has resulted in gaps in drug safety data, which can lead to unrecognized risks, inappropriate medication avoidance, or delayed guideline recommendations. ja.ma/3RtMWGs

💬 Editorial: Virtual and remote #ClinicalTrials can expand access and capture real‑world outcomes, but they require careful design to address data quality, patient safety monitoring, and participant engagement without in‑person contact. ja.ma/48GLPsR

Patch-based remote ECG screening for atrial fibrillation in older individuals with moderate to high risk of stroke modestly increased diagnosis and anticoagulation but had limited effect on #stroke rates. ja.ma/3P03bdu

In patients with early septic shock, the ANDROMEDA‑SHOCK‑2 randomized clinical trial evaluated a personalized hemodynamic resuscitation protocol targeting normalization of capillary refill time. Compared with usual care, this strategy was superior for a hierarchical composite outcome of mortality, duration of vital support, and length of hospital stay at 28 days, with the overall benefit primarily attributable to a shorter duration of organ support. 🧵
