へぇー！IL-6R遺伝子多型（SNPs）🧬が片頭痛に関係しそうなのか😳 この論文で調べたSNPsは… ❶ −183G/A（rs4845617）：IL-6R遺伝子のプロモーター領域 ❷ 48892A/C（rs8192284）：IL-6R遺伝子のエクソン9 の2つ✌️ それらを ・前兆のある片頭痛（MA） 63例 ・前兆のない片頭痛（MO） 113例 ・緊張型頭痛（TTH） 35例 ・健常対照（CTL） 62例 で調べると、 ✅❷ のCアレルはMAで有意に多かった ✅MOやTTHでは有意な関連はなし🙅‍♂️ ✅❶は頭痛との有意な関連はなし🙅‍♂️ ちなみに❷は、IL-6Rの切断部位に影響し、膜結合型IL-6Rから可溶性IL-6R（sIL-6R）が生じるシェディングに関係すると考えられています 実際に48892CアレルはAアレルよりsIL-6R濃度が高いって報告されていますしね それでsIL-6Rが増えるとIL-6シグナルが強まって炎症反応、血管反応性、中枢性感作、疼痛感受性が高まる可能性があるってことか🤔

This study reports 48-month real-world eptinezumab outcomes in a highly refractory cohort. • Multiple exposure-defined cohorts (all-initiator, on-treatment, continuation, discontinuation) were used to address the complexity of long-term real-world treatment trajectories. • Symptom improvement was modest and most apparent among patients who remained on eptinezumab, while overall treatment persistence declined over time. • Post-discontinuation pathways were heterogeneous, with frequent switching and re-engagement with anti-CGRP therapy, reflecting iterative preventive management in chronic migraine. journals.sagepub.com/doi/10.… #migraine #headache #brain #pain #neurology #neuroscience

脳外科医は知っておきたい。 頭部外傷後頭痛の薬物治療のレビュー📚 
「で、何を処方するのが正解？」🤔
聞かれると、よく分からない領域です。 
今回のレビューでも、外傷後頭痛に対して
「これを第一選択で全員に使う」
と言えるほど強いエビデンスの薬剤はなさそうです。 
限られた報告からでいうと、、
・片頭痛様 →CGRP薬、トピラマート、ボツリヌス
・神経障害性疼痛様 →ガバペンチンなど
・脳震盪後症候群に伴う頭痛 →アマンタジンなど うーん、結局は頭痛の表現型に合わせて、
個別化して考える必要があるのですね🤔
