A request to hospital clinicians: if you have never worked in a SNF (skilled nursing facility), please be humble when telling patients what to expect when they go there. It's OK to say you don't know.

Fighting against antibiotic resistance and changing hospital culture around "UTIs" for older adults can feel like a losing battle. Godspeed @h0spitalista
New patient list today and I am very disheartened to say 4 patients on my census are 90 Fs with mild dementia admitted for delirium. All started on abx in ER and continued by my colleagues on admission. None of the 4 had GU symptoms, fever, or even "appeared ill" Worse yet:

Almost every week I meet a patient moving from hospital to nursing home w/ plans to pursue low benefit rehab Not because of personal choice Because Medicare will cover their nursing home expenses if they do rehab but not if they elect hospice Can we please do better?

Nursing facility rounds today on several pts with end stage disease. Orders written for miralax, stop the vitamins, do not hospitalize. For refractory muscle cramps, tonic water (placebo?). For the spirit, gin in the tonic water (a negotiated dose). A good day's work.
