This was an excellent podcast. There were many important points. Something that resonated strongly with me about our current culture was the observation that "making an effort to understand is assumed to be pointing a finger of blame" and the pattern of "conflating curiosity with blame". It has been especially scary, and destructive, to see this pattern in our medical institutions and "professional" organizations.

I'm a bit annoyed that NYT has settled on the title “Here’s What Psychiatrists Mean When They Say You Have A.D.H.D.” (presumably because it got more engagement) rather than the prior title “We’re Thinking About Mental Health Diagnoses All Wrong,” because the piece isn't really about ADHD. It's just an example I used in one passage.
When patients ask, “What disorder do I really ‘have’?” the honest answer is usually more interesting and messier than a single label. I wrote for the @nytimes on what I wish people understood about diagnoses and the nature of mental health problems. nytimes.com/2026/05/11/opini…

I hate the phrase "dopamine hit" - dopamine is about incentive salience and prediction error, not a literal "shot of pleasure" every time you see a notification or eat a cupcake. Let’s stop turning complex neurobiology into a buzzword. DOPAMINE DOESNT WORK THAT WAY.

The people who behave in this manner do a disservice to everyone, including those who are actually autistic. A big contributor to the collective knee-jerk reaction of distrust and skepticism towards "autism" is due to these self-appointed diagnostic evangelists.
Almost without fail, after I share my BPD recovery story, strangers slide into my DMs to inform me I'm actually autistic. They never keyboard diagnose me with anything else. But they never tell me what criteria they're using to make this determination. It's almost like they think autism is a catch-all diagnosis. Where they got that idea, I haven't the foggiest.

Self-interpersonal psychopathology finds cover and refuge in whatever diagnosis it can, whatever diagnosis will allow the person to avoid confronting the nature of their pain and the self-repeating patterns of their lives, whatever will make for a more appealing & dignified story
A lot of people are mistakingly diagnosing personality pathology as Autism and anyone who has spent time in autism communities see them come in and wreck absolute havoc 😵

You are starting with the assumption that psychotherapy is about“therapist-guided practices” and “adherence.” That’s not my understanding of psychotherapy.

I think most clinicians would agree that the session itself needs to remain a protected human space. Where things get interesting is outside the room — many clients leave with meaningful insight but struggle to retain or apply it without some kind of structure between sessions. The question may not be whether AI belongs in therapy, but whether it can responsibly support adherence to therapist-guided practices once the session ends.
