A ‘never event’: wrong side cerebral angiogram
Pt had R MCA occlusion. Thrombectomy planned. In angio, got a LICA injection, clot absent, assumed due to thrombolytics, procedure ended. Error discovered, pt returned to angio at delay for R thrombectomy.
