In September 2016, we called on hospitals from across Ontario to join us in implementing a patient oriented discharge summary (below). 21 hospitals did, and 42k patients and 4 years later, the results are in. ncbi.nlm.nih.gov/pmc/article…

We are working with patients and partners @UHN to develop content for a #Covid_19 specific PODS - coming soon. Key considerations are to continue to involve patients and families as essential partners in care when reviewing instructions on how to manage at home
