Pelt, MD, points out a common disconnect: hospitals may not know how to assign real cost to SPD time, tray burden, employee overtime, and case volume. Difficult bacteria to treat. No good oral antibiotics other than Linezolid which has cost and toxicity issues. Unique resistance patterns can occur with Corynebacterium as well.
Corynebacterium-associated PJIs represent a unique subset of disease that can be challenging to manage, but with close collaboration between surgical and infectious disease teams, reasonable outcomes can be expected. arthroplastyjournal.org/arti…

Cementless TKA is solved? Shorter OR time, potential longer term fixation (?), use of robotics, and promising outcomes w/ some newer f designs. But... We saw a trend that the while the tibia has largely improved, the femur may be the implant of concern. sciencedirect.com/science/ar…

Curious to hear - have you seen cementless TKA with femoral sided failures? Do you look at that side routinely?

Could alignment strategies, or bearing designs have an influence? Is there a difference with more congruent bearings vs flat bearings or PS? A lot is left to learn. But perhaps just having the awareness that we may need to look at the femur will help us look at this further.

Maybe bone doesn't have the same affinity for ingrowth into CoCr as it does Ti?

We don't know all the reasons. But have a few thoughts. Some implants use HA or PA, and some do not. And the femurs are usually CoCr. CoCr is a little stiffer - stress shielding or stress relaxation of bone under the implant?

In this study, the team adds to paucity of existing literature on the femoral side in cementless TKA. 66% of RLL were seen on the femoral side. 83% of the revisions for aseptic loosening were for the femur. @ArthroToday
