"For instance, a digital health record (DHR) - providing a single view of the patient for all clinicians - would seamlessly link community-referral pathways and allow an entirely new model of outpatient care to emerge. While a fully functional NHS App would offer allow patients to better manage their own health and offer better access to the care they need, when they need it. "We must create a health system fit for the 21st century - that should start and end with the patient - but it must be driven by data.”
