Pure retroperitoneal ureteral access using the low anterior approach with da Vinci SP is a powerful and versatile technique for managing a wide range of ureteral strictures. These procedures can often be performed with PACU discharge and are more efficient and less morbid than transperitoneal approaches to the ureter. SP Retro Pyeloplasty: The Basics I'm often asked by new SP users how easy it is to manage crossing vessels during pyeloplasty with the low anterior approach. This is one of the biggest advantages over traditional multiport retro where transposition of the ureter anterior to a crossing vessel can make suturing the anastamosis challenging. Not so with low anterior. Here are before and after images from a right sided dismembered pyeloplasty. Easy suturing!

Be careful to assess whether the contralateral kidney's collecting system extends across the midline in the isthmus of a horseshoe kidney. This case demonstrates a robotic left horseshoe heminephrectomy with exposed dilated right-sided collecting system in the transected isthmus that was closed in two layers. This is rare and occurs in < 5% of cases but if you miss this feature, can result in a troublesome urine leak.

Outpatient STEP procedure in 89 year old with 450 - 500 g prostate without prominent median lobe. 330g of prostate resected, 3 day catheterization. Although not a great case for early in your learning curve, very straightforward with modest experience provided you stick to disciplined and meticulous technique. Brute force is not the way!

In an era of outpatient regionalized single site robotic surgery, how are you treating distal ureteral strictures (example shown). Transvesical reimplant? Retroperitoneal side to side reimplant? Something more traditional? What’s your rationale?
