The MBTA is required by law to report any fatalities to oversight agencies within two hours of the incident.
Those agencies says they didn't know a man had died after getting caught in an escalator until we published an investigation

When a death was reported as a "fall" on an escalator, Ryan quickly requested the surveillance video. Without that instinct — and that speed — this story would never have been told.
