Late to this party because I'm over on Le Ciel Bleu more now, but my fave for open surgery is the insulated, teflon coated diathermy blade tip. Used properly it can cut, coag, dissect, and be used as a teaching pointer- and comes in endless extensions.

Yes- my fav for open thoracotomy as well. The Matson-Alexander is the best tool for harvesting an intercostal muscle flap, which is the most underappreciated flap we have. ICM flap can butress, rebuild tracheal wall, close esophageal leaks, and interpose to separate structures.
