Multivisceral transplant is tricky. Donor OR doesn’t need porta dissection but needs accurate dissection from retroperitoneum. Knowledge from HPB, Colorectal, trauma surgery is needed. The Treiz / Toldt fusion fascia is one of the most important points to handle operation.

So lucky to learn from my brilliant senior fellow, @ToruGotoMD ! An amazing surgeon and educator.
Replaced CHA or RHA from SMA makes deceased donor operation complicated. Our strategy and safe operation for total replaced CHA from SMA. The bifurcation was 4cm from Ao and retrograde dissection from hilum towards SMA was done. Warm dissection time took only 1.5hr as usual.
