This week’s case comes from an 84-year-old male who was referred to the haematology team after routine blood tests showed a markedly raised white cell count. Clinically, he reported several weeks of general fatigue but no acute symptoms. He was otherwise well and presented via his GP.
The FBC showed:
- White cell count: 823 ×10⁹/L (Note: This value exceeded the analyser’s linearity range)
- Red cell count: 2.69 ×10¹²/L
- Haemoglobin: 84–106 g/L (range reflects correction due to WBC interference)
- Haematocrit: 0.306
- MCV: 92.4 fL

A 59 year old presented to the ED with fatigue, shortness of breath, and spontaneous bruising.
Laboratory results showed:
- WBC: 168 × 10⁹/L
The increased white cell count prompted a blood film review (see images below).
A sample was sent for flow cytometry and molecular analysis. You can see the results of those investigations on the link above.

✅Symptoms: fatigue, back pain, recurrent infections
✅Circulating plasma cells
✅Mild rouleaux formation
✅Anaemia and pancytopenia
✅Increased total protein
Diagnosis: multiple myeloma
While plasma cells on a film raise suspicion, distinguishing multiple myeloma from plasma cell leukaemia isn’t always straightforward, especially with limited clinical details and mild rouleaux.
In this case, the patient's symptoms and raised total protein levels helped confirm the picture of multiple myeloma.

- Fatigue and pallor
- Easy bruising
- Reduced urine output
- Mild abdominal pain following GI illness
Their FBC result showed:
- Hb: 113g/L
- WBC: 5.46 ×10⁹/L
- PLT: 16 ×10⁹/L
A blood film was made due to markedly low platelet count. Images are attached.

This week's case is from a 6 year old with chronic anaemia jaundice splenomegaly.
Some of their laboratory results showed:
- Hb: 78 g/L
- MCV: 55 fL
- MCHC: 365 g/L
- Reticulocytes: 9%
- Total Bilirubin: Elevated (predominantly indirect)
- LDH: Raised
- Direct Antiglobulin Test (DAT): Negative
