Days like today serve to emphasise that the pilot's job is not about the times they make things work against the odds but about the times they say "no" or "stop"... or, "go-around"
And always have a review before proceeding 😇
Confirmation bias is essentially seeing what we want to see rather than whole picture, potentially leading to wrong decision or actions At safety critical times, worth pausing & checking that we have all the information available/required & asking others for input too if unsure

On average we make 5-7 mistakes a day - error is a 'normal' part of being human Reducing them by planning ahead is part of threat & error management Could be easy to get time difference wrong from UK for HF talk tmw night in New Zealand. So checking it now to avoid any issues!

Tunnel vision is one example of losing situational awareness (SA) We can totally lose track of time because of being task focused & hours past by without realising Having a good team looking out for each other is a great way to build SA Empower everyone to do so at team brief!

Earlier this year, Sage predicted that Covid hospitalisations would reach 60,000 by July. In reality, that number was just 932 yesterday. Why are we letting them influence whether or not restrictions lift on 21 June? Instead, they should focus on fixing their broken modelling.

A very simple and transferable idea... happy to have played a small part
Team briefing is vital before major surgery or any safety critical procedure But when things don't go to plan its best to stop for a mini-brief Just had paper accepted in @RCSnews about our PPP method 1) Patient 2) Procedure 3) People (team) (or changed order as required)
