Premenopausal women with ACS are often labeled “low risk.” The reality: rising MI rates, more delays and misdiagnoses, and worse outcomes vs men.
ACS in younger women is different and needs tailored care, earlier recognition, and equity in treatment.

ALT Screening • Use validated instruments to screen for depression and anxiety (eg, BDI-II, PHQ-9, GDS, HADS) • Optimal timing of screening is uncertain but could be conducted at intervals post-ACS • Consider referral to mental health care professional Diagnosis • Confirm diagnosis of depression or anxiety, or both, prior to initiating treatment Treatment • Use shared decision-making to inform treatment decision • CBT and antidepressant mediations (eg, SSRI) are options and may be used together for optimal effect Re-Assessment • Reassess depression or anxiety, or both, symptoms and necessity of continued treatment or modification to treatment plan

In another analysis, we found a significant burden of rare variants in FMD-associated genes (e.g., F11R, TLN1) in SCAD patients compared to controls.
Key takeaway: SCAD is rarely "monogenic" but heavily influenced by a cumulative burden of rare variants.
