.@drgoodyear is pointing at something bigger than slowing a tumor down. He's describing reprogramming. A dedifferentiated liposarcoma grows faster and hides better from the immune system. Walk it back toward a well differentiated state and you change both its metabolism and its visibility to immune cells. The drug matters less than where you put it. Systemic GLP-1 acts on the whole body. Injected into the lesion, it does local work without the systemic cost. That is the heart of intratumoral therapy. Change the cell, then let the immune system finish.

The Iowa study doubled survival by adding IV vitamin C to first line chemo, 8 months to 16. We take it further and inject the vitamin C straight into the tumor, same as we do with the KRAS inhibitors. The compound is rarely the problem. Delivery is, and what it does to the immune response.
