That's a concept art from Bio2, possibly a reference or tease. I suspect that scenario r004 (which are the sewer areas of EoTR) were to connect with the area we see in the intro of File #1.

He could've secretly given Manuela to TerraSave or had Barry take her in. Many alternatives. It'd have spared her Sherry's misery (worse for Manuela, she has no Claire). Leon generally thinks he knows best, especially for women. It's a flaw of his since BIO2.

She wasn't a "main character" to begin with, in fact she was meant to die in BIO2. How playable she is doesn't affect the plot. She could be not playable at all & it'd be the same.
