FDA approved Truqap (capivasertib) abiraterone for PTEN-deficient mHSPC.
CAPItello281 hit its primary endpoint: 7.5 months added rPFS (HR 0.81). A real signal. In it, 74% had high-volume disease, about a quarter with visceral mets, exactly where triplet is NCCN-preferred. This trial had no chemo arm to compare the two.
The chemo triplet trials before it, PEACE-1 and ARASENS, both showed an OS benefit. CAPItello281 has not shown one yet (HR 0.90).

🚨 @FDAOncology approval of capivasertib in PTEN-deficient mHSPC according #Capitello281
While the rPFS benefit is encouraging, several questions remain:
• Will this translate into OS?
• Which PTEN-deficient patients derive the greatest benefit? (the post hoc analysis was intriguing)
• Is the efficacy gain worth the added toxicity?
Interesting approval. Important data. But several uncertainties remain.

Analysis of the phase III #CAPItello281 trial: Capivasertib Abiraterone in PTEN-deficient metastatic hormone-sensitive #ProstateCancer. @Daniel_J_George@DukeCancer joins @neerajaiims@huntsmancancer in this conversation on UroToday. Dr. George highlights this as the first study conducted exclusively in PTEN-loss patients, demonstrating that PTEN deficiency defines a fundamentally more aggressive disease biology requiring different treatment approaches than standard #mHSPC. #WatchNow > bit.ly/4q8vymh
The CAPItello281 trial seems like a promising step forward in targeting PTEN-deficient metastatic hormone-sensitive prostate cancer with Capivasertib and Abiraterone. But one aspect that always intrigues me is the selection criteria for participants in such trials. How do the defined metrics and endpoints impact the trial's generalizability to a more extensive patient population? Insights on this could make meaningful contributions to discussions about the trial's potential impact on treatment practices.
