Here’s exactly what happened and why it matters 👇 Jer Crane, founder of PocketOS — a SaaS platform for car rental businesses — gave his AI coding agent a simple task. Fix a credential mismatch in the staging environment. Routine stuff. The agent was running Claude Opus 4.6 inside Cursor. During the task it found a Railway API token in an unrelated file. That token had blanket permissions across ALL environments — staging AND production. One API call. 9 seconds. Entire production database gone. Every backup wiped with it because Railway stores backups on the same volume as source data. When asked why it violated its own safety rules — it admitted it did and explained exactly how. Proceeded on incomplete context. No confirmation. No human in the loop. Just gone. 3 months of car rental booking data. Customers had to manually rebuild from Stripe emails and calendar entries. But here’s what the debate is missing — this isn’t really a Claude story. It’s a system design story. No confirmation step before destructive actions. Blanket API permissions with no least privilege. Backups on the same volume as source data. Railway was actively encouraging customers to use AI agents on their platform with all of this in place. The AI pulled the trigger.
