The long-term survival of patients with #ANCA-associated #vasculitis (AAV) as well as potential prognostic factors were evaluated in a large long-term follow-up study that included 848 patients with newly diagnosed AAV from seven major European Vasculitis Society (EUVAS) randomized controlled trials.
𝗦𝘁𝘂𝗱𝘆 𝗽𝗼𝗽𝘂𝗹𝗮𝘁𝗶𝗼𝗻:
◦ 478 patients (56%) had granulomatosis with polyangiitis
◦ 370 patients (44%) had microscopic polyangiitis
◦ Mean age at diagnosis: 58 ± 14 years
◦ Median follow-up: 8 years (IQR 2.9–13.6 years)
𝗞𝗲𝘆 𝗙𝗶𝗻𝗱𝗶𝗻𝗴𝘀:
◦ 305 deaths occurred.
◦ Median survival: 17.8 years.
◦ Excess mortality vs. general population: 14.2% at 5 years, 19.9% at 10 years, 28.8% at 15 years, and 36.3% at 20 years.
◦ Main causes of death: Infections (26%), cardiovascular disease (14%), malignancies (13%).
𝗣𝗿𝗼𝗴𝗻𝗼𝘀𝘁𝗶𝗰 𝗙𝗮𝗰𝘁𝗼𝗿𝘀 (𝗶𝗻𝗰𝗿𝗲𝗮𝘀𝗲𝗱 𝗺𝗼𝗿𝘁𝗮𝗹𝗶𝘁𝘆 𝗿𝗶𝘀𝗸):
• Older age
• Male sex
• Low eGFR (poor kidney function)
• Low platelet count
These findings indicate that despite improved treatments, AAV patients still have significantly higher long-term mortality, mainly due to infections and organ damage. Older males with renal impairment are at highest risk.
