🚨🎗️ ASCO26 | OPTIMA Trial Supports Chemotherapy De-escalation in Genomically Low-Risk HR /HER2− Early Breast Cancer

The phase III OPTIMA trial demonstrated that the 50-gene Prosigna® (PAM50) assay can identify a subgroup of clinically high-risk HR-positive/HER2-negative early breast cancer patients with minimal benefit from adjuvant chemotherapy.
✅ 4,153 patients enrolled
✅ HR /HER2− early breast cancer
✅ Up to 9 positive lymph nodes included
✅ Median follow-up: 4.0 years
✅ Primary endpoint: Invasive Breast Cancer-Free Survival (IBCFS)
Among patients classified as low molecular risk (ROR ≤60):
✅ 5-year IBCFS: 93.6% with endocrine therapy alone vs 94.8% with chemotherapy endocrine therapy
✅ HR 1.06 (90% CI 0.80–1.40)
✅ Non-inferiority achieved (p=0.003)
✅ Absolute difference at 5 years: only 1.2%
✅ Similar outcomes across menopausal status, tumor size, grade, and nodal burden
✅ Consistent results observed even in patients with 4–9 positive lymph nodes and stage IIIA disease
✅ Premenopausal women receiving ovarian function suppression also maintained excellent outcomes
The investigators concluded that Prosigna identified a population with little, if any, meaningful chemotherapy benefit. Based on the observed results, chemotherapy would prevent at most approximately 2 recurrences per 100 patients classified as low genomic risk.
These findings further support the growing role of genomic profiling in refining adjuvant treatment decisions and safely reducing chemotherapy exposure in selected patients with clinically high-risk HR /HER2− early breast cancer.
