FDA approves belzutifan with pembrolizumab for adjuvant treatment of renal cell carcinoma

The FDA 🇺🇸 has approved belzutifan in combination with pembrolizumab as adjuvant therapy for patients with clear cell renal cell carcinoma (ccRCC) at intermediate-high or high risk of recurrence following nephrectomy, including those with resected metastatic disease. The approval is based on the phase III LITESPARK-022 trial, where the combination significantly improved disease-free survival versus pembrolizumab alone (HR 0.72; p=0.0003), reducing the risk of recurrence, metastasis, or death by 28%. Overall survival data remain immature, while safety findings were consistent with the known profiles of both agents.

The FDA has approved capivasertib in combination with abiraterone and prednisone as the first targeted therapy for patients with PTEN-deficient metastatic androgen pathway modulation-naïve or sensitive #ProstateCancer . Approval was based on the Phase III CAPItello-281 trial, which demonstrated a 19% reduction in the risk of radiographic progression or death and improved median radiographic progression-free survival by 7.5 months. The decision also highlights the growing importance of biomarker-driven treatment and PTEN testing in advanced prostate cancer.

In a real-world cohort of 457 patients, nivolumab ipilimumab chemotherapy was associated with longer OS than pembrolizumab chemotherapy in PD-L1 TPS <1% tumors:
• Median OS: 47.4 vs 16.6 months
• HR 0.50 (p=0.007)
No OS difference was observed in PD-L1 TPS ≥1%.

Talquetamab daratumumab ± pomalidomide beat DPd after ≥1 prior line:
📌 24-mo PFS
Tal-DP: 81.3%
Tal-D: 77.6%
DPd: 51.2%
HR 0.28 and 0.33. Both P<0.001.
Depth was striking:
MRD-negative CR: 52.3% / 46.3% vs 15.9%
⚠️ Key toxicity signal:
CRS common, mostly low grade.
Taste, skin/nail changes, weight loss and ataxia need attention.
My take:
This is not just “another triplet.”
It may redefine early relapse myeloma by bringing GPRC5D targeting much earlier.
