It was an absolute pleasure and privilege to join a distinguished panel late morning morning today on Your World on NTV, graciously hosted by the ever-energetic Winnie Lubembe, as we marked the culminating week of the Cervical Cancer Awareness Month. I was delighted to share the platform with my outstanding colleagues Dr Kireki Omanwa, (Consultant Obstetrician & Gynaecologist, University of Nairobi lecturer, and President of the Kenya Obstetrics & Gynaecology Society KOGS); Prof Omenge Orang’o (Gynaecologic Oncologist, HPV / Cancer researcher and Aga Khan University lecturer); and Millicent Kagonga, a cervical cancer survivor whose lived experience and advocacy powerfully grounded the discussion in real-world impact. The talk show focused on accelerating cervical cancer elimination through awareness, prevention, early detection, screening, vaccination, timely diagnosis, treatment, and survivorship. I was particularly glad to contribute the laboratory and pathology perspective, emphasizing the importance of HPV testing being the recommend testing instead of Pap smear or VIA to promote accurate diagnostics, increased uptake from self-sampling and well-integrated care pathways for facilitating easier access to screening & treatment. The NTV team @ntvkenya has done well to create such an important platform to educate and reach our communities, especially around primary prevention through HPV vaccination that is ideally for girls aged 9–14, but beneficial for women up to 45 years - as well as HPV testing for cervical cancer screening, including the empowering option of self-sample collection. Cervical cancer is virtually 100% preventable through vaccination and screening. Let us continue to spread this message far and wide to our families, our communities, and every woman who needs to hear it. American Cancer Society Updates Cervical Cancer Screening Guideline: Major Changes Include Self-Collection for HPV Testing and Guidance on Exiting Screening 米国の子宮頸がん検診指針を改定　自己採取HPV検査を容認し終了基準も明確化 2025年12月4日、米国のAmerican Cancer Societyは、平均リスクで子宮頸部を持つ人の子宮頸がん検診指針を更新した。 新たに、自己採取した腟スワブをHPV（ヒトパピローマウイルス。 ほぼすべての子宮頸がんに関わる）検査に使うことを認め、医療機関に行きにくい人の受診機会を広げる狙いがある。 採取は医療者が行う方法が望ましい。 検診は25歳から65歳まで5年ごとのHPV単独検査が基本で、難しい場合はHPV＋細胞診（パップ検査）を5年ごと、または細胞診のみを3年ごとに行う。 自己採取で陰性なら3年後に再検査する。 終了条件も明確化され、60歳と65歳でHPV系検査が陰性であることを確認してから中止する。 New American Cancer Society guidance (Dec. 4, 2025) allows self-collected vaginal samples for HPV testing, helping people who can’t easily get to clinics. Age-specific clinical performance of HPV-based vs. cytology-based cervical cancer screening in China 中国における子宮頸がん検診：HPV検査は細胞診より年齢を問わず見逃しが少ない 中国の9施設で21〜64歳の女性28,501人を対象に、一次検査としてのHPV検査（陽性なら細胞診で絞り込み）と、細胞診（異常ならHPV検査で絞り込み）を比較した。 初回検診では、CIN2 （中等度以上の前がん）を見つける感度がHPV法で96.9%と、細胞診法の79.7%より高かった一方、特異度は88.8%で細胞診法の92.7%よりやや低かった。 3年間の追跡でもHPV法の感度は93.1%で、細胞診法の58.1%を大きく上回った。 年齢別でもHPV法の高い感度は維持された。HPV16/18陽性の3年CIN2 リスクは43.0%で、HPV陰性は0.2%だった。 結果は、HPV検査の長期的な安心感と検診間隔延長の可能性を示す。 In 28,501 Chinese women, primary HPV testing with cytology triage detected CIN2 far better than cytology with HPV triage at baseline (96.9% vs 79.7% sensitivity) and over 3 years (93.1% vs 58.1%), with slightly lower specificity.
