Strengthening last-mile access to primary healthcare, Smile Foundation, in partnership with HDFC Bank under its CSR initiative Parivartan, has launched 4 Mobile Care Units (MCUs) in Uttarakhand. The units were officially flagged off by the Hon’ble Chief Minister of Uttarakhand, Shri Pushkar Singh Dhami, at the CM House in Dehradun. During the interaction, the Government appreciated the initiative and encouraged its expansion to reach more underserved areas across the state. These MCUs aim to deliver essential primary healthcare services at the doorstep of communities, ensuring timely access to consultations, basic diagnostics, and preventive care in regions where access remains limited. Launched as part of HDFC Bank Parivartan’s commitment to last-mile healthcare, the project focuses on bridging critical gaps in regions where terrain and limited resources often restrict access to essential services. Alongside treatment, the programme will also promote preventive healthcare through awareness sessions on hygiene, nutrition, and overall well-being. We thank HDFC Bank Parivartan for partnering with us in this journey of reaching more lives with every mile and spread. #HealthCannotWait

Smiles begin with good health. Families, primarily farmers from some of the remotest villages in Ramanathapuram district of Tamil Nadu, receive free healthcare services right at their doorsteps through Smile Foundation’s Smile on Wheels programme. Working as daily wage earners and marginal farmers, these villagers toil relentlessly to feed their families, often neglecting their own health needs. The Smile on Wheels project reaches remote villages in the district with essential health services, including OPD consultations, diagnostic support, medicine distribution, and referral services. In addition, health counselling and awareness sessions on various preventable diseases are regularly conducted for families. #HealthCannotWait

Healthcare access isn’t only about distance. It’s about reaching people where they are. For many families in Pune, care often begins late when it becomes unavoidable. Primary healthcare is not something people turn to early but something they reach for in urgency. Small symptoms are often overlooked and routine check-ups are rare. With support from Athenahealth, Smile Foundation’s Smile on Wheels is helping change this. Early screenings can prevent conditions that often go untreated for years influencing both outcomes and everyday health-seeking behaviour. #HealthCannotWait

Awareness is the first step towards managing hypertension. Through Smile on Wheels, regular hypertension outreach continues to reach villages and underserved communities in Madurai, bringing essential information and early detection services closer to people who need them most. As part of the mobile healthcare project’s routine visits, community members were screened for blood sugar, blood pressure and related indicators to help identify possible warning signs at an early stage. Those with high readings were guided for timely medical follow-up. Simple counselling sessions helped people understand what hypertension is, how to recognise early symptoms and how everyday habits, nutritious meals, physical activity, stress control and regular monitoring, can support better management. I am 70 years old. These days, my knees hurt every time I walk. My body feels tired even before the day begins. I used to manage everything in the house, but now even small chores feel heavy. There are four of us—me, my husband, my son, and daughter-in-law. No one in the house is earning right now. We manage with whatever little help we get. I have never worked outside, only looked after the family. And now, when I need support, it’s not easy to ask. Smile on Wheels comes close to our area. I can go there without spending anything or troubling anyone. The doctors check me properly and give me medicines that ease the pain. I may not be able to do as much as before, but this care helps me stay on my feet a little longer. At this age, that means a lot. At my age, even a small health problem can feel heavy. My husband is also not keeping well, my son works long hours, and my daughter-in-law manages the home. I never wanted to trouble them to take me all the way to the hospital for regular checkups. When Smile on Wheels started visiting our area, it made things so much easier. I can get my blood pressure and sugar checked, take my medicines on time, and talk to the doctor without hesitation. They explain everything with patience and even give simple tips on how to take care of our health at home. It may seem like a small thing, but for me it has taken away a big worry. I feel cared for without feeling like I am adding to my family’s responsibilities. Rukmani Devi, 57 is a regular visitor of Smile on Wheels, mobile healthcare unit in Kurla, Maharashtra. Bringing healthcare to river-bound villages in Assam In the remote riverine islands of South Salmara, Assam, where villages are scattered and often surrounded by water, timely access to healthcare remains a daily challenge. To bridge this gap, Smile Foundation, in partnership with MSD, is operating Smile on Boat - a mobile boat clinic reaching communities beyond the access of regular health services. The boat clinic is serving 12 riverine islands, catering to over 27,000 people, including women, children, and the elderly, with essential primary healthcare. Along with curative care, the focus is equally on spreading health awareness and encouraging health-seeking behaviour among the villagers. From antenatal check-ups to regular follow-ups, from reducing out-of-pocket expenses to strengthening maternal and child health practices, the initiative is helping build a culture of preventive care, one visit at a time. Aligned with the goals of the National Health Mission, Government of India, Smile on Boat is working to ensure last-mile healthcare delivery with dignity and consistency. “We are a small family of four; my husband, our two boys and me. My younger one is just a month old and between caring for him and managing the home, my days are always full. My husband is the only one earning, so we plan every expense carefully. Going all the way to the city for a check-up is not easy. It takes time, money and someone has to watch the children. That’s why Smile on Wheels helps me so much. The doctors come to our area, check on my health after delivery and guide me on how to recover well. They also sit with us and talk about many things, how to eat better after delivery, why it’s important to keep the baby’s surroundings clean, how to prevent common infections and even simple tips to stay healthy as a family. I have learned things I never really thought about before, like the importance of regular check-ups and small changes at home that can keep my children safe from illness. When the van comes, I feel reassured. I don’t have to ignore my own health while looking after my family and I feel more confident that I’m doing the right things for my children too.” S. I am a daily wage worker in Madurai. Work is uncertain. Some days I get called, some days I don’t. But I have to keep showing up. For months now, I’ve had pain in my knees. Even standing for long hours has become difficult. I feel weak all the time, but I kept ignoring it. Going to a hospital means spending money I can’t afford and losing a day’s wage. Then I heard about Smile on Wheels, a medical van that comes to our area. I went one morning. The doctor checked me, gave medicines, and some advice. I didn’t have to pay or travel anywhere. Now, I go every time the van comes. It helps me keep going because stopping is never an option for people like me. I’m 74. I live alone and manage things on my own; cooking, cleaning, going to the market when I have the energy. Every 15 days, I visit the Smile on Wheels clinic that comes near the temple. They check my BP and sugar, and I get my regular medicines. The doctor knows me by name now. It makes things easier. I don’t have to travel far or wait in long lines. Old age comes with its own set of problems. But having a place I can go to, where I’m not rushed, where someone listens. That helps more than people realise. “For a long time, I suffered from dizziness and headaches. Later, the doctors told me it was because of high blood pressure. But at this age, going for regular check-ups and medicines is difficult. Then these doctors and nurses came to our village—and everything changed. Now, I don’t have to worry about walking miles. The doctors and nurses come right to my doorstep. They check my blood pressure, give me the medicines I need and explain everything with so much love and patience. They listen patiently, explain things in simple words, and make sure I never miss a dose. I feel safer now. My headaches have reduced, I sleep better and I feel stronger each day. At my age, health problems are many. We elderly people often ignore them because there is no help nearby. Thank you to the team who brings not just medicines, but also peace and smiles into my home.” Raimuni Soyam, Chaibasa, Jharkhand To join hands in our efforts, please visit: donate.smilefoundationindia.… #HealthCannotWait #SmileOnWheels
