担当医師の許可というより個人情報保護法上 個人情報を外部提供するため患者の同意が必要です。 ID-LinkやHumanBridgeのような地域医療連携ネットワークは患者の同意を取得して連携する形になっています。そのための事務コストが非常にかかっています。

地域医療情報連携ネットワークとは ちょっと異なっているというのがあって…… 地域医療情報連携ネットワークは 所謂ID-LINKとかhumanbridgeと呼ばれている 仕組みを使っていく事で情報の共有化を 目的としていたのですがこれは運用に 乗せられれば強いんですがそうとは限らなかったと。

As one of the most prominent local and international charitable organizations, the Barzani Charity Foundation (BCF) has dedicated itself to serving every person in need. Working in close collaboration with local and international organizations, foundations, and donors, BCF operates across a wide range of humanitarian sectors. One of the key areas is food security. The foundation provides essential food supplies to those suffering from hunger, including refugees and displaced people. For example, BCF delivered emergency food aid to victims of the devastating earthquakes in Syria and Turkey. Another major focus is shelter and protection. Following the conflicts in Syria and Iraq, thousands of people fled to the Kurdistan Region in search of safety. BCF teams welcomed them, provided immediate necessities, and resettled them in various locations both inside and outside refugee camps. The foundation also plays an important role in cash assistance and education, supporting orphans and vulnerable children to ensure they have the care, resources, and opportunities needed for a dignified life. Special emphasis is placed on helping children continue their education and secure a brighter future. BCF has also contributed to building, renovating, and expanding classrooms across many cities in the Kurdistan Region. Another critical sector is non-food assistance, which includes distributing household items, medical supplies, construction materials, and other essential goods to families affected by natural disasters such as earthquakes, floods, and fires or man-made crises like conflict and displacement. The health sector is equally vital, with particular priority given to women and children. BCF provides medical treatment both within the Kurdistan Region and, in urgent cases, abroad. In addition, its mobile medical teams regularly visit villages to deliver healthcare services wherever they are needed. These examples represent only a small part of the Barzani Charity Foundation’s work. Guided by the words of General Mullah Mustafa Barzani, who said “It is an honor for every human being to serve their nation,”. BCF continues its mission to support humanity with dedication and compassion. NOBORIを筆頭にクラウドPACSは中規模以下でまあまあ普及しています。画像データの発生元の撮影装置はオンプレ側（院内側）にあるので院内に小型の中間サーバーだけ置くなど色々な工夫をしてますね。 ID-LinkやHumanBridgeがDICOMビューアを内蔵しているようにブラウザだけで動作するビューアもあります

ID-LinkやHumanBridgeのようにリアルタイムに医療機関のDICOMサーバーにDICOM Q/Rすれば技術上は可能ですがシステム導入費用がかなりかかりそうです。全画像をクラウドに上げれば回線容量とストレージ容量の問題が‥

閉域網の電子カルテの情報を他でも見たい という動きが「地域医療連携ネットワーク」 と呼ばれているシステムだ。 電子カルテが異なるメーカーでも その情報を閲覧可能にする仕組み。 実は2000年代からある仕組みだったりする。 NECのID-LINKや富士通のhumanbridgeが代表的。

現状、紹介状（診療情報提供書）を電子的にやりとりするなら👇のようなソフトで紹介状のPDFにHPKIで電子署名してID-LinkやHumanBridgeのようなセキュアな地域医療共有ネットワークを使用して相手先に提供する必要がある。 orcamo.co.jp/products/signed…

ID-LinkやHumanBridgeのような 地域医療情報連携ネットワークでは 画像も含めて医療情報を共有できますが 導入費用や患者同意を取る事務コストが大きく広くは広まっていません。 現在モデル地区で開始された厚労省の 電子カルテ情報共有サービスでは診療情報提供書の電子的なやり取りも実装されます。

ID-LINKとかhumanbridgeとか 触った事ある人ならイメージ付きやすいけど 電子カルテ内に情報が来るというより 電子カルテにボタンが追加されて それを押すとブラウザ（chromeやedgeなど）が 開いてそこで検査や画像が見れるという イメージが近いかな。
