推移パターンの可視化には inverse probability of censoring weighting（IPCW）を用い、さらに6年後の最終状態の推移確率の算出には多項ロジスティック回帰モデルを用いました。 これにより、保険脱退による打ち切りの影響を考慮しながら、フレイルの長期的な推移パターンを評価しました。

TROPION-Breast02 (Ann Oncol April 2026) 免疫療法非適応の未治療進行TNBC（n=644）に対する1次治療として、TROP2指向性ADCであるDato-DXd（datopotamab deruxtecan；6 mg/kg、3週毎）vs 医師選択化学療法を比較。データカットオフ：2025年8月25日、中央値追跡期間27.5ヶ月。 対象患者： PD-L1低発現（CPS<10）、またはPD-L1高発現でも免疫療法が使用不可（早期乳癌での prior IO後再発・合併症・規制上のアクセス不可）の局所再発切除不能または転移性TNBC。転移性TNBC患者の約70%がこの未充足集団に該当。 主要エンドポイント（2項目とも達成）： 中央値PFS（BICR）：10.8ヶ月 vs 5.6ヶ月 — HR 0.57（99%CI 0.44–0.73）；P<0.0001 中央値OS：23.7ヶ月 vs 18.7ヶ月 — HR 0.79（95.01%CI 0.64–0.98）；P=0.029 副次エンドポイント： ORR：63% vs 29%（CR：9% vs 2%） 奏効期間中央値：12.3ヶ月 vs 7.1ヶ月 安全性： Grade≥3 TRAE：33%（Dato-DXd）vs 29%（化学療法） TRAEによる投与中止：4% vs 7%（Dato-DXd群で低率） Dato-DXdの主な副作用：口内炎57%、悪心45%、脱毛41% ILD/肺炎：3%；Grade 5イベント1例（担当医は乳癌関連死と判断）. 両群とも治療関連死亡なし 注意点： OS改善は米国・カナダ・欧州サブグループでは一貫せず（HR 1.22、95%CI 0.86–1.73）。この地域では化学療法群での後治療ADC使用が多く（70% vs 31%）、後治療差を補正したIPCW調整OS HRは0.90（95%CI 0.54–1.51）。OS利益の地域差と後治療の影響に注意が必要。 免疫療法を使用できない転移性TNBCという最大の未充足ニーズに対し、TROP2指向性ADCとして初めてPFS・OSの両主要エンドポイントで有意な改善を実証。ただし後治療のアクセス状況が地域によりOS解釈に影響する点は留意すべき。 doi.org/10.1016/j.annonc.202… #乳癌 #TNBC #ADC

治療中止をちゃんとモデル化できてれば(未観測交絡の話は常に付きまとうとして)良さそうな気もするだけど、どうなんだろう。IPCWが普通に使われるならそれと同じ話な気がして。
