🔬 Gram stain from a positive blood culture showing Nocardia
Typical Gram-positive, branching, beaded filamentous rods are seen — a classic microscopic clue for Nocardia spp.
🩺 Patient: SLE with fever and weight loss
🧬 Final ID: Nocardia carnea by VITEK MS
💡 Teaching Pearl:
In immunocompromised patients, early recognition of branching Gram-positive rods should immediately raise suspicion for Nocardia. Prompt communication with clinicians allows early targeted therapy and appropriate culture handling (prolonged incubation, alerting lab staff, biosafety awareness).
🔍 Differential Reminder:
✔️ Nocardia → Aerobic, weakly acid-fast, beaded branching filaments
✔️ Actinomyces → Anaerobic, non–acid-fast, dense branching filaments, often associated with sulfur granules
A simple modified acid-fast stain can help distinguish them rapidly.
📌 Microscopy still matters.

MedMicro (MM) consultant kindly reminded me not to compare myself, a struggling Peds ID fellow, with smart MM residents who could identify an organism based on colony morphology alone; MM resident would probably also feel the same way if rotating in Peds ID. We're all learners.

A patient with PAH on epoprostenol via a central line develops a catheter-related bloodstream infection. What bacteria , has been linked to such infections? 🤔 #IDTwitter#MedMicro#MedEd
My @PurdueBiolSci BIOL 533 #MedMicro class did an outstanding job on their posters today!
Students were assigned articles extending from class content. They also designed a "next experiment" based on their article & evaluated if #AI helped refine the expt.

Our next speaker after our lunch break was Dr. Chris Doern from VCU Pathology. Dr. Doern presented valuable data on navigating blood culture utilization amidst the ongoing bottle shortage. #medmicro

Mean salary of MedMicro/ID in Quebec is ~530k I think
Canadian $
Minus ~40-50% taxes
We’re not soooo far I think 🤔
But then comes the discussion about cost of living, services with our taxes and USA president
Goood evening @jcomls
I'm a 4th year Medlab student, specialising in ChemPath.
Been your follower for a while now and I have been learning from your post, I can conclude you are Medmicro specialist.
Pls can you advise me on any topic or new research in Chempath for my seminar
Embraced #AI in my #BIOL533#MedMicro class. Students used @OpenAI Chat-GPT as aid to understand primary literature & designed follow-up experiment based on class lectures. They then defended their experiment in a public poster session. Great exercise & turnout!

I learned this in MedMicro! It’s a pretty unique transmission where it originates from an infected rodent/animal (infected by flea bite) which can spread to human, and that infected human can infect other humans! Caused by bacteria Yersinia pestis.
