Immunothrombosis in hospitalized COVID-19 patients identified by multiomics profiling and linked to postacute complications 🚨INTERESTING New Latvian/Swedish multi-omics study shows immunothrombosis never fully switched off in longC0VID patients: 3 months after COVID hospitalization, your blood is still biologically “clot-ready.” ➡️What makes this study so important: 1. It reinforces that immunothrombosis (the interplay of complement, NETs, and platelets) is a central driver in severe acute COVID-19 and can persist in longC0VID, 2. It confirms persistent endothelial Dysfunction/Endotheliopathy in longC0VID patients, consistent with earlier studies on vascular damage and microclots, 3. It aligns with prior evidence of mitochondrial dysfunction during the acute phase, followed by partial repair mechanisms, 4. The persistent complement activation they observed fits with other recent multi-omics studies showing ongoing complement dysregulation in longC0VID. ➡️Study: 1. Prospective longitudinal multi-omics study of 81 hospitalized COVID-19 patients tracked whole-blood transcriptomics, urine metabolomics (46 analytes), and 13 kidney-injury biomarkers at acute admission, ~1 month, and ~3 months post-hospitalization, 2. Patients stratified by EHR into recovered (n=35) versus long COVID (n=46) groups based on a PASC diagnoses within 12 months, 3. None of the 81 hospitalized patients were vaccinated, 4. Acute phase dominated by interconnected immunothrombosis: strong upregulation of complement (C1QA/B/C), NETosis (PADI4, MPO), and platelet-activation genes (ITGA2B, ITGB3), plus mitochondrial dysfunction (HIF1A/EPAS1 up, OXPHOS down, Warburg-like glycolysis) and elevated renal injury markers (KIM-1 etc.), 5. Most immune, mitochondrial, and metabolomic changes largely normalized by 1–3 months, with rebound in mitophagy/heme genes (PINK1, OPA1, FECH) indicating repair, 6. At 3 months, longC0VID patients showed a distinct transcriptional signature of persistent endothelial activation (↑VWF, PROS1, ITGA2B/ITGB3), complement dysregulation (CFH), and low-grade vascular inflammation/platelet reactivity (CXCL5, ALOX12) that was absent in recovered individuals, 7. Was sind NETs ? Grok : NETs = Neutrophil Extracellular Traps (Neutrophilen-extrazelluläre Traps / Neutrophilen-Netze). Einfache Erklärung Neutrophile sind die „Spezialeinheit“ deines angeborenen Immunsystems – die häufigsten weißen Blutkörperchen. Bei einer Infektion oder starker Entzündung können sie eine dramatische Abwehrreaktion starten: Sie schleudern ihre eigene DNA (zusammen mit Histonen und aggressiven Enzymen wie Myeloperoxidase/MPO oder Neutrophilen-Elastase) nach außen und bilden damit ➡️ klebrige, netzartige Strukturen – die sogenannten NETs. Diese Netze wirken wie ein Spinnennetz im Blut oder Gewebe: Sie fangen Viren, Bakterien oder Pilze ein und töten sie dort ab. Das ist eine sehr effektive Waffe der angeborenen Immunität. Warum können NETs problematisch werden? Normalerweise werden NETs nach ihrem Einsatz wieder abgebaut. Bei Überaktivierung (starke NETosis) oder gestörter Auflösung entstehen jedoch Schäden: • Starke Entzündungsreaktionen (Zytokinsturm) • Aktivierung der Blutgerinnung → Mikrogerinnsel (oft fibrin-amyloidartig, wie in manchen Long-COVID-Beobachtungen beschrieben) • Schädigung der Gefäßinnenhaut (Endothelschäden) • Dysregulation des Komplementsystems • Gewebeschäden in Lunge, Herz, Niere, Gehirn etc. Genau diese Effekte werden in der von dir genannten Spike-Tox-Hypothese als zentrale Mechanismen genannt. Was sagen die Studien zu COVID-19 und Spike? Mehrere peer-reviewed Arbeiten zeigen: • Bei COVID-19-Patienten sind Marker für NETs (z. B. MPO-DNA-Komplexe, citrulliniertes Histon H3) im Blut deutlich erhöht und korrelieren mit Schweregrad, Thrombosen und schlechter Prognose. • Das SARS-CoV-2-Spike-Protein selbst (sowie das Nucleocapsid-Protein) kann direkt die Bildung von NETs in gesunden Neutrophilen auslösen. Eine Studie von 2021 wies das explizit nach: ➡️ Spike induziert NETosis – und zwar ROS-unabhängig (also nicht über die klassische oxidative Stress-Schiene). • Auch das intakte, vermehrungsfähige Virus 🦠 kann Neutrophile direkt zur NET-Freisetzung anregen. Das passt sehr gut zu den von dir beschriebenen Effekten: Endothelschäden, Mikrogerinnsel, anhaltende systemische Entzündung und Multiorgan-Beteiligung bei akutem COVID und Long COVID. Parallelen zu Schlangengiften (wie in der Hypothese beschrieben) Einige Schlangengiftkomponenten (besonders sPLA2, 3FTx inkl. Cardiotoxine/CTX und bestimmte SVMP) können ebenfalls NETosis auslösen oder sehr ähnliche pathologische Effekte hervorrufen (Membranschäden, Endothelschäden, prothrombotische Zustände, Entzündung). Eine konkrete Studie zu Viperngift (Echis carinatus) und Kobragift (Naja naja) zeigte, dass beide Venome menschliche Neutrophile zur NET-Bildung anregen – allerdings über teilweise unterschiedliche Mechanismen (ROS-abhängig vs. ROS-unabhängig, teilweise über 3FTx-Cytotoxine). Die Hypothese „Spike als toxinähnliches Analogon“ nutzt genau diese mechanistischen Überschneidungen, um zu erklären, warum ein virales Protein so breite, anhaltende Schäden verursachen kann. Zusammengefasst: NETs sind eine mächtige, aber potenziell zweischneidige Waffe des Immunsystems. Im Kontext von SARS-CoV-2 und der Spike-Tox-Hypothese tragen sie wesentlich zu den beobachteten Mikrogerinnseln, Endothelschäden und systemischen Entzündungsprozessen bei – und es gibt direkte experimentelle Hinweise, dass das Spike-Protein diese NETosis mit auslösen kann.//
