Clinical case: 63-year-old patient with presyncope and dyspnea. Holter monitoring revealed a high PVC burden (25%). Antiarrhythmic therapy was not tolerated due to severe bradycardia. Successful PVC ablation was performed via a retrograde aortic approach using the CARTO navigation system. Activation mapping identified the earliest activation at the left coronary cusp, with immediate PVC suppression.
