My cousin, who is in his late 40s, developed Transverse Myelitis three years ago. Before that, he was an extremely active person and someone the entire family looked up to for advice and guidance on almost every matter. He runs his own business and also oversees businesses belonging to other relatives. By nature, he tends to overthink and takes time when making decisions. Around the same period, he was also building a house. Construction has been ongoing for nearly five years now, with a long pause when he was bedridden following his illness. Although he has recovered significantly, he has not returned to full health. One side of his body remains weak, and it continues to affect his daily activities. The finishing work on the house has been dragging on for the last 7–8 months. His parents and siblings often complain that he is not actively involved anymore and that the delays are largely due to his lack of enthusiasm and follow-up. He too agrees with that assessment. He admits that he often feels tired and reluctant to do anything. He says he experiences a sense of relief whenever something scheduled for the day gets cancelled. My own view is that the burden of responsibility in his family has never really been shared. He has parents, a younger brother, a wife, a brother-in-law, and several other family members around him, but very little of the responsibility seems to be taken on by anyone else. When he was healthy, he was able to carry that load without much difficulty. Now, however, his physical condition does not allow him to function at the same level. To me, he appears to be showing signs of depression and anxiety, but there is no one around him who can explain this to him in a way he might accept. He is also from a generation and mindset that is not particularly open to discussions about mental health. I have tried speaking to him about it, but the age difference makes it difficult to have that kind of conversation freely, and his response was largely resistant to the idea. At the moment, he seems to be dealing with multiple pressures simultaneously..ongoing physical limitations, financial responsibilities, business commitments, and a house construction project that never seems to end. While he continues to say that everything is fine, I am not convinced that he is coping as well as he believes.

India’s TB Shield: NTEP’s Proven 6-Month ATT Still the Standard Saving Millions —————————————————- In India, the National Tuberculosis Elimination Programme (NTEP) continues to use the standard 6-month Anti-Tubercular Therapy (ATT) as the prevalent, evidence-based treatment for drug-susceptible pulmonary tuberculosis. This regimen — two months of intensive phase with isoniazid, rifampicin, pyrazinamide and ethambutol (HRZE or equivalent fixed-dose combination), followed by four months of isoniazid and rifampicin (HR) — remains the national standard under directly observed therapy. Decades of large-scale implementation data show this regimen reliably eradicates Mycobacterium tuberculosis when adhered to, forming the backbone of India’s TB control efforts and contributing to millions of cures worldwide. While 2025 international guidelines (ATS/CDC/ERS/IDSA) conditionally recommend a shorter 4-month regimen (2HPZM/2HPM with rifapentine and moxifloxacin) for eligible adults with drug-susceptible pulmonary TB — validated as non-inferior in major randomised trials — India’s NTEP has not yet adopted it as the default. Programmatic factors including cost, drug availability and large-scale rollout logistics keep the trusted 6-month ATT as the prevalent option. For multidrug-resistant TB, NTEP has introduced shorter 6-month all-oral regimens (BPaLM) for eligible patients. These advances come solely from microbiology, pharmacology and rigorous clinical evidence — not dilutions, juices or untested remedies. NTEP’s 6-month ATT continues to deliver high cure rates in India’s high-burden setting, proving that targeted, tested science remains the only approach that consistently eradicates TB and saves lives on a massive scale. I think we shall all quit @claudeai and @AnthropicAI, turn off our subscriptions and move into Chinese open source AI models, more honest prices and same if not better performances for some model, and paradoxically, more freedom than USA is nowadays capable to provide. This action masked as a mandate to obedience, it’s just showing how weak can a state be when still attempting to control, manipulate, hide, and fear humanity for what’s capable to do with love and creativity. While on the other hand, they’ll use those very same tools for war, crime against humanity, and control for the seek of power. Blockchain and decentralisation was just the beginning, now it’s going to be about inference and ai models.
