Presented by Peter Voorhees at the #EHA2026 Plenary Session, #MonumenTAL-3 evaluated whether introducing talquetamab-based combinations earlier in the treatment course could improve outcomes for patients with relapsed/refractory multiple myeloma. 🎀 The INAVO120 safety analysis brings an important practical message: efficacy matters, but manageability determines real-world use. Published in ESMO Open, this analysis evaluated the safety profile of inavolisib plus palbociclib–fulvestrant in patients with PIK3CA-mutated, HR-positive, HER2-negative, endocrine-resistant advanced breast cancer. The INAVO120 regimen had already shown improved PFS and OS. This analysis focused on how clinicians can manage key selected adverse events: 📌 Hyperglycaemia 📌 Stomatitis/mucosal inflammation 📌 Rash 📌 Diarrhoea Most of these events occurred early, especially during the first treatment cycles, and were generally manageable with supportive medications and dose modifications. Key safety points: ➡️ Hyperglycaemia was mostly grade 1–2 and commonly managed with metformin ➡️ Stomatitis/mucosal inflammation was often managed with dexamethasone mouthwash ➡️ Rash was mostly low grade, with no grade 3–5 events reported ➡️ Diarrhoea was usually short-lived and commonly managed with loperamide ➡️ No new or unexpected safety signals were observed The takeaway: INAVO120 supports the use of inavolisib plus palbociclib–fulvestrant as an effective regimen with a safety profile that can be managed through early monitoring, patient education, supportive care, and dose modification. 🎀 A HER2 bispecific antibody has now challenged the global first-line standard in HER2-positive advanced breast cancer. Alphamab Oncology announced that the phase III KN026-003 trial met its primary endpoint. The study evaluated anbenitamab (KN026) plus HB1801, an albumin-bound docetaxel, as first-line treatment for HER2-positive advanced breast cancer. The comparator was the current standard first-line regimen: ➡️ trastuzumab ➡️ pertuzumab ➡️ docetaxel also known as THP According to the company, KN026 plus HB1801 significantly prolonged progression-free survival versus THP, with a statistically significant and clinically meaningful benefit. The study also showed a trend toward overall survival benefit and a favorable safety profile. Why this matters: KN026 is designed to bind two non-overlapping HER2 epitopes, domain IV and domain II, aiming to block HER2 signaling more comprehensively than conventional single-target antibody approaches. This is a major topline signal for HER2 bispecific antibody-based therapy in breast cancer. The full dataset is still needed. Key questions remain: 📌 What was the median PFS? 📌 What was the hazard ratio? 📌 How strong was the OS trend? 📌 Which subgroups benefited most? 📌 How did safety compare with THP? 📌 Could this support a “first-line bispecific, second-line ADC” sequencing strategy? 🫁 In PD-L1-negative advanced NSCLC, the choice of first-line immunotherapy combination may matter. A multicenter retrospective cohort study published in Lung Cancer compared two widely used first-line regimens in advanced NSCLC without actionable driver mutations: ➡️ Nivolumab ipilimumab chemotherapy ➡️ Pembrolizumab chemotherapy The analysis included 457 patients treated across 13 institutions in Japan. Patients were stratified by PD-L1 tumor proportion score: 📌 PD-L1 TPS <1% 📌 PD-L1 TPS ≥1% The strongest signal came from the PD-L1-Negative Advanced NSCLC cohort. With follow-up exceeding 40 months, nivolumab plus ipilimumab with chemotherapy showed longer overall survival than pembrolizumab with chemotherapy: 📌 Median OS: 47.4 vs 16.6 months 📌 Adjusted HR: 0.50 📌 36-month OS rate: 51.5% vs 28.2% In patients with PD-L1 TPS ≥1%, survival outcomes were similar between the two regimens. Safety was broadly comparable, although severe skin toxicity and adrenal-pituitary disorders were more frequent with nivolumab plus ipilimumab. The takeaway: For PD-L1-Negative Advanced NSCLC, dual checkpoint blockade plus chemotherapy may provide a sustained long-term survival advantage — but prospective validation is still needed.
