#StressEcho showing ischemia in a 70-year-old woman (stress-induced chest pain ST-segment changes wall motion abnormalities). Still, coronary angiogram was normal.
Up to 20% of patients with myocardial ischemia may have INOCA.
CIAO-ISCHEMIA will tell us more.

why does Pharma exclude 'poor prognosis' patients from trials?
It comes down to 💰. If pts stop due to comorbid reasons, it adds more deviation in the outcome measure. So they need a larger trial to show statistical significance.
Should we require sponsors to include PS 2/3, Hgb<9, real-world pts in Phase 3 trials?
