Due to unforeseen circumstances an Easter Egg appears to have fallen out of the cupboard and opened itself. Therefore, in order to avoid food waste I have eaten it. I shall be making no further comment about this upsetting accident and ask that my privacy is respected. Thank you.

Q3: one of the most common barriers I've seen to quality improvement over the years is leaders losing interest or energy or moving on to other priorities. Most large scale change efforts fizzle out.
