What are the possible mechanisms linking short and long sleep durations with cardiometabolic outcomes? Is it possible for very short sleep duration to result in a proteomic fingerprint that is selectively associated with one cardiometabolic disease but not another? Our study identified 16 unique proteomic markers that were associated with sleep durations. When compared to a sleep duration of 7.21-7.57 hours, short sleep duration (4.00-6.57 hours), was linked with upregulation of markers of inflammation, apoptosis, and cell adhesion, and with downregulation of markers related to angiogenesis and resolution of inflammation. When combining these markers into a proteomic score based on their individual weights, we found that this score was significantly and positively associated with incident diabetes mellitus (DM) but not with incident coronary heart disease (CHD). However, individuals who developed diabetes during the course of the study were still at a significantly higher risk of CHD. The findings suggest that very short sleep duration has a proteomic fingerprint that is specifically associated with incident DM. It may also suggest the following pathway as one of the links between short sleep and incident CHD: Short sleep duration --> inflammation and apoptosis --> incident DM --> incident CHD Although long sleep duration has been linked with several adverse health outcomes including incident DM, our study did not identify an independent association between the markers predictive of the longest sleep duration and incident DM or incident CHD, thereby suggesting alternative pathways and mechanisms for such associations. doi.org/10.1186/s12916-024-0… #sleep #proteomics #Diabetes #inflammation
